Annual Faculty Report
Each year in the beginning of the Fall Semester members of the faculty are asked to complete the Annual Faculty Report (AFR), which chronicles faculty accomplishments and contributions across the full range of teaching, research and scholarship, service and outreach activities. We rely on the AFR as the best (and, with respect to some activities, the only) source of information about faculty accomplishments. The AFR provides the official record on which many faculty personnel decisions are based, and it is vital to faculty development, both as an opportunity for self-reflection and as a basis for discussion among departmental and other colleagues. It also represents a unique opportunity to inform the society we serve of our accomplishments, so it is our interest to be sure that what we do is fully and fairly reported.
Every person, full or part-time, holding the rank of Lecturer, Instructor, Assistant Professor, Associate Professor, Professor, Head or Chair, or Dean should fill out these forms. Teaching Associates and Assistants are not required to fill out the forms.
A Word copy of the form will be sent to you at the start of the Fall semester.
You may also go to the Provost Office site and download a Web-Based form. If you choose this method, please remember to create an electronic copy of this form for us before hitting the send button!
If you are using a template from last year, please remember to update the year.
AFRs should be submitted to Laura Wildman-Hanlon in an electronic format – preferably in Word, not PDF form. She will print out a hard copy for your signature. She will also provide you with a copy for your records.
Please remember to include an updated copy of your CV with your AFR, also in electronic form.
Sabbatical Leave or Leave of Absence Requests
The new fillable sabbatical forms (the application and the approval form) are now up on the Provost’s website. The link to their site is: http://www.umass.edu/provost/faculty-staff-resources/personnel-information. Unfortunately, I cannot give you a separate link to the forms themselves but you can follow the link to the Provost website page where the forms are located. You will also find there the Sabbatical Policy chart. Please contact me if you have any questions on your eligibility.
Periodic Multi-Year Review
PMYR is conducted every seven years for all tenured faculty members. Information regarding the PMYR process may be found in Appendix A and Article XXXIV of the MSP bargaining contract. A copy of the Periodic Multi-Year Review Policies can also be found on the Provost Office web site.
If PMYR reports include a plan for teaching enhancement, additional information is available on the Center for Teaching website
A Request for PMYR Funds Form, completed and signed, should be submitted with your PMYR materials.
